Insurance Benefits
DreamHammer pays a significant percentage of all benefits if an employee elects to receive our benefits program.
Medical Insurance
We offerDreamHammer-supplemented national medical plans at both the PPO and HMOlevel through United Healthcare (UHC). In California PacifiCare,KaiserPermanente and Blue Cross of California replace UHC. In Hawaii BlueCross of Hawaii replaces UHC HMO. The DreamHammer-offered plans provideemployees and dependents the opportunity to choose between in-networkand out-of-network care and services. The Plans offer an extensivenetwork of providers, hospitals and facilities. DreamHammer employeesare eligible for health insurance benefits upon the first day of hire.
Dental Insurance
Weoffer one national dental plan through United Healthcare. TheUnited Healthcare Plan provides employees and dependents the opportunityto choose in-network or out-of-network care and services.United Healthcare offers an extensive network of providers. Our Planincludes an orthodontia benefit.
Vision Insurance
Weoffer one national vision Plan through VSP. The VSP Plan providesemployees and dependents the opportunity to choose in-network orout-of-network care and services. This plan provides coverage forroutine exams, glasses and contacts. Because it is considered electivesurgery, laser vision correction is not a covered benefit under ourplan. However as a member of our plan, an employee of DreamHammer canreceive discounts on laser eye surgery including LASIK and PRKprocedures.
Short Term Disability/Long Term Disability
DreamHammer offersa national disability Plan through CIGNA. With respect to Short TermDisability, after meeting a 14 day elimination period, the plan benefitpays 60% of the employee's weekly base salary, up to a maximum benefitof $2,308 per week, for up to 180 days. Once short term disability ends,our long-term disability benefit pays 60% of the employee's monthly basesalary, up to a maximum benefit of $10,000 per month, up to age 65.
Additional Disability Insurance
At theirown expense, employees have the option of purchasingadditional individual disability income insurance that provides evengreaterincome protection.
Life Insurance
DreamHammer employeesautomatically receive a life insurance policy of up to 1xSalary(maximum of $50,000) provided by the Company. We also offer a voluntarylife insurance plan through CIGNA which DreamHammer will subsidize as afringe benefit for our employees up to an additional 1xSalary.Additionally, at their own expense, an employee may elect to purchaseadditional life insurance coverage for themselves and their dependents.
Employee Leave
Comprehensive Leave
Eligible employeesaccrue vacation time for personal business and enjoyment and sick timefor short-term illness each pay period. Accrual rates vary,based uponyears of service with the Company.

Most DreamHammeremployees are eligible for eleven holidays during the year. DreamHammertraditionally observes the following holiday schedule:

DreamHammer offers morethan 1200courses in career skills and software on-line free of chargeto all employees. DreamHammer offers a Tuition Reimbursement Program,in addition to career development opportunities, with an annual ceilingof$1,500 per employee. This benefit covers bothdegree-related($1,500/course maximum) and short courses and/orcertifications($500/course maximum) taken by an employee, on their owntime, and related to their position with the Company.
Employee Referrals
DreamHammer employeeswho refer individuals personally known to them for open positions areeligible for bonuses of $500 - $1,500 if the referral is hired.
